If you need help with a medical problem, we will ask you to complete a quick and easy form which you can find on our website.
If you don’t have access to a computer, you can still ring the surgery and we will complete the form for you.
All forms will be reviewed by a GP within 24 hours. Our aim is to improve access and support patients to get the right help from the right professional in the right timeframe.
