Primary Care Network Team
The practice is supported by a number of clinical staff who work across the 3 practices in Didcot as part of the Didcot Primary Care Network.
Clinical Pharmacists
This team are responsible for managing patients with complex medication regimes, changes in medication following discharge from hospital and a number of long term conditions where optimisation of medication is essential. The team also provide clinics to support patients with medication queries including alternatives for out of stock items or alternative formulas when swallowing tablets might be difficult. They also run clinics for heart failure and raised blood pressure.

Physiotherapists
This team will see patients with musculo-skeletal problems to identify an initial course of treatment or signpost for longer-term management and treatment.

MIND Counsellors
Our MIND counsellors provide talking therapy to give you space to talk about your thoughts and feelings and help you find ways to make sense of things and/or change the way you think or respond. They will also provide support in signposting you to other counselling services which may support and suit your individual needs better. We also have a young person’s MIND counsellor who will support parents and young people with services and talking space when they need it.

Social Prescribers
Our social prescribers work for partnership organisations including Age UK, Sofea and MIND. They are there to support patients with a range of social issues which may be impacting on their health including loneliness, support with benefits and funding needs, access to sufficient and healthy food, challenges and difficulties at home.
